To my patients,

Due to unacceptable contracting by OhioHeath Clinically Integrated Network, I am closing my practice, and will no longer be available to provide you medical care effective November 26^th, 2025.  I will be available until that time for your health care needs. Below are a few gynecology practices that I would recommend, but do not feel limited by this list. Check your insurance to see if they are in your network.

I am including a medical record request form if you feel you need copies.  Realize that all surgeries, deliveries, pathology, Pap smears, cultures, biopsies and labs are in the OhioHealth Epic system.  If you need copies of your office medical records (there will be a charge for copies and mailing – Please see request form for details).  After December 12^th, 2025 your medical record will be placed in storage, and to request copies please follow the instructions on the enclosed medical record request form marked “After December 12^th, 2025” instructions.

In parting, thank you for the honor and privilege of allowing me to take care of you. You are family. I have loved getting to know you, and helping you understand your physical and mental health. Thank you for your trust, and I wish you a long, healthy, and happy life.
